It appears that if there are benefits of using adenosine receptor antagonists to decrease tubuloglomerular feedback-mediated vasoconstriction and increase renal blood flow and GFR in AKI, they may be limited to very specific populations (e. g., asphyctic neonates). These benefits must be balanced against potential adverse drug effects: both renal (increased renal blood flow and distal salt delivery might harmfully increase tubular oxygen consumption in the presence of ATN), and nonrenal (lower seizure threshold). Thus, further studies are still needed to clarify the role for theophylline in neonates.
